This request is for help in getting a punching bag for a 14 year old teenager. This young person struggles with his emotions. He has made many impressive strides. He was advised to buy a punching bag as it would be a productive way for him to express his feelings. However, the family cannot afford a punching bag. This young person would like to start boxing one day, but due to covid-19 it is not possible at the moment. Contact Jessica Hansen at jrhansen@jeffco.us to assist you. Mychart Scl Health.

COVID Vaccine Resources
Here is the State of Colorado website/page that contains ALL the information AND registration links to receive the COVID-19 VACCINE, 70 and older (in PHASE 1B), by county.

If anyone needs help navigating the registration process for any of these sites, they can call or email Benefits In Action at 720-221-8354 or info@benefitsinaction.org for assistance.

Providers for people aged 70 and over:

Many Colorado hospital systems currently schedule vaccination appointments for Coloradans age 70 and older who are eligible for vaccination during Phase 1B — and many more are working to move online. In some cases, hospitals make contact with their patients. In other cases, patients can register at these hospitals to be contacted about a vaccination appointment. The speed at which hospitals can vaccinate Coloradans age 70 and older depends on the federal supply of vaccines. Because we try to act as quickly as possible, we provide information as soon as it is publicly available. Please understand that they will evolve, change and grow over time. The following list describes how patients of these providers can sign up to be notified when a vaccine is available.

JEFFERSON COUNTY

Centura Health
If you are a patient at Centura Health, you will receive an invitation via MyCenturaHealth, the patient portal, when it is time to receive the vaccine. These invitations will be sent out according to the state system, starting with patients who are 70 years of age and older. It takes time to get a covid-19 vaccination. Walk-in is not possible. Use the vaccine notification registration form to receive additional vaccine information and instructions via email and/or text. Colorado residents can take action now by contacting a Centura Health Physician Group primary care physician in their area.

Kaiser Permanente
For Kaiser Permanente members age 70 and older (and others in eligible categories 1A and 1B), visit the Kaiser Permanente Immunization webpage to complete the “COVID-19 Vaccine Registration.” After submitting your information, you will be placed on a vaccine waiting list based on your eligibility.

SCL Health

SCL Health is currently contacting patients aged 70 years and older who are currently receiving care at an SCL Health facility. Information will be emailed to these patients on how to use the MyChart online patient portal to schedule a vaccination appointment. These invitations will be sent out in phases based on the state level system.
Individuals interested in getting vaccinated may consider creating an SCL Health MyChart account for the fastest and easiest experience.
To receive the covid-19 vaccine at an SCL Health facility, an appointment is MANDATORY – entry vaccinations are NOT available. If you are age 70 or older and would like more information about registering for the COVID-19 vaccination at an SCL Health facility, log on to the SCL Health website or call 303-812-2051 for more information.

As part of its mission to improve the health of the people and communities that SCL Health serves, especially the poor and vulnerable, the goal is to vaccinate as many members of their communities as possible quickly and safely in accordance with the booths’ guidelines.

DENVER COUNTY

Denver Health
Denver Health is currently working to contact and schedule appointments with patients ages 70 and older who are currently being treated at a Denver Health facility or have been treated in the past three years.

National Jewish Health has partnered with Intermountain Health (formerly SCL Health) to offer MyChart to our patients. MyChart is a patient portal used by many healthcare organizations in the United States. It has advanced features that make it easier to access your health information and care from your smartphone, tablet or computer.

On May 1, 2023, we transitioned from our old My National Jewish Health portal to MyChart. All patients, including those with a My National Jewish Health account, must log into MyChart to access their health information and receive care online.

Mychart Scl Health

If you already have an active MyChart account with Intermountain Health and are a patient with National Jewish Health, your MyChart username and password will give you access to your health information for both organizations. Please note that after May 1, 2023, you will only see health information for visits to the National Jewish Health Clinic.

Do I need a National Jewish Health MyChart account if I have an account from another health organization?

Unless your MyChart account is with our partner Intermountain Health (formerly SCL Health), you will need a separate account for National Jewish Health.

How do I link my National Jewish Health MyChart account to an account with another health system?

Video transcription

Unfortunately, we cannot transfer any information—including your username and password—from My National Jewish Health to MyChart. You must register for a MyChart account on or after May 1, 2023.

The My National Jewish Health portal will remain open with limited, read-only functionality until November 30, 2023. It contains clinic visit information through April 30, 2023.

To access a family member’s health information through MyChart, you must request proxy access. Read more.

Why can’t I access my health information in MyChart before May 1, 2023?

Unfortunately, neither our current system nor MyChart allows us to transfer health information between systems. Your doctor will continue to have access to your files until May 1. Your care will therefore not be interrupted during this transition.

How can I send messages to my provider in MyChart?

In MyChart, you can send messages to healthcare providers with whom you have had an appointment (in person or via video) in the past year or with whom you have scheduled an appointment within the next 180 days. Click here to download instructions for posting.

If you are having trouble finding your provider in MyChart, call 303.398.1355, option 4 to message your clinical team.

What is care everywhere?
With Care Everywhere, your healthcare providers have secure access to your medical records from other healthcare organizations.

Your national Jewish health care providers share information when they need to coordinate care for you. Data is automatically updated between care providers over the secure Care Everywhere network. Your healthcare provider will see your information from other healthcare organizations directly in your medical record. This allows them to meet your care needs better and more efficiently. If you do not want your information to be shared between health care organizations, you can opt out by completing this form.

Who do I contact if I have problems accessing my MyChart account?

Contact us at 1.303.398.1355, option 5.
